Residents of the Municipality of Anchorage (MOA) are required to submit a health permit application for the MOA Cottage Food program to prepare foods in a kitchen for sale directly to customers. (MOA 16.60.105)

Tips for completing the health permit application form:
- Check the box at the top of the form that says "Cottage Food".
- Fill out the document with accurate name and contact information (Phone, Address, and Email).
