​​​​How to Apply

Applying for WIC is an easy 5 step process:


Step 1:

Submit a Family Information Form.

Step 2: ​

Submit an application for each person you are applying for:

Step 3:

Once you submit a Family Information Form, you will receive an email with a link to a location to securely upload your proofs of income, ID, and residency eligibility.  Please follow the instructions in the email to submit your information.


We accept the following for proof of income:​
  • Denali Kid Care
  • Medicaid
  • Last 30 days pay stubs with gross pay
  • W2 and past year income tax return documents
  • ATAP letter
  • Food Stamps/SNAP letter
  • LES
  • Unemployment letter
  • Child support
  • OCS foster care paperwork​

We accept the following for proof of ID:
For parent or guardian:
  • Driver’s License
  • Military ID
  • Passport​

For child:
  • Crib card
  • Temporary Proof of Birth
  • Birth Certificate
  • SSN card
  • Immunization Shot Records
  • Current Denali Kid Care card​

We accept the following for proof of residency:
  • Alaska Driver’s License
  • JBER Orders
  • Bill/mail with current address 
  • Paystubs​
  • Current Denali KidCare card

Step 4: 

Once the WIC clinic receives your FIF, application and proofs, we will call you to schedule your WIC certification appointment. You can also call us at 907-343-4668 to follow up regarding your applications.

Step 5: 

​​Complete your WIC certification appointment and start to receive benefits! Most of our appointments are over the phone but there is the option for an in person appointment if needed.
