Teen Nightclub/Cultural Performance Venue Permit

NOTE: Applications must be received 90 days prior to event or operation.

  • AMC 10.55.010 regarding Teen Nightclub/Cultural Performance Venue Permit
    • Term:  1 Year (from the day of issue)​
    • Fee:  New $200 or Renewal $100​ (Paid to MOA)​
    • Background Check Fee: $20 (Check/Money Order ONLY - Paid to Department of Public Safety)

Provide the following with your application:

  •  Proof of Insurance
 As required by AMC 10.55.065
Must be current.
  • Proof of Permit for Unlicensed Nightclub
  • Parking Diagram

Must obtain Unlicensed Ni​ghtclub Permit from Planning Department per AMC 21.05.050D.8.c.​

Required for applicant, officers, directors, partners, employees, or any other person involved in the business operation. Subject information must be completed and signed. 

  • ​​​Provide copy of government issued i​dentification.
  • Include payment in the form of cash or a ch​eck made out to Department of Public Safety.​
