ELEVATOR APPLICATIONS- for Minor Alterations or Modernizations/Upgrades.  IF YOUR PROJECT FALLS INTO ONE OF THOSE 2 CATAGORIES THE FOLLOWING INFORMATION IS REQUIRED TO BE FILLED OUT ON THE COMMERCIAL WORKSHEET.

  1. IS THAN AN EPLANS (ELECTRONIC) SUBMITTAL?
  2. Construction Site Address, PARCEL ID, LEGAL DESCRIPTION, OR STREET ADDRESS.
  3. OWNER NAME
  4. Permittee, Contractor, Contact Person
  5. PROJECT NAME/Description of Work - Fill out Description of work under Commercial Alterations section

Minor Alterations or Modernizations/Upgrades: THE FOLLOWING INFORMATION IS REQUIRED TO SUBMIT.

  1. IS THIS AN EPLANS PROJECT?
  2. PARCEL ID, LEGAL DESCRIPTION, OR STREET ADDRESS.
  3. OWNER NAME
  4. EPLANS CONTACT- NAME, EMAIL AND PHONE NUMBER.
  5. CLEARLY FILL OUT THE DESCRIPTION OF WORK AT THE BOTTOM OF THE FORM.
  6. PROJECT NAME

FEES FOR REMODELS, TENANT IMPROVEMENTS ARE CALCULATED ON CONSTRUCTION VALUATION.  CONSTRUCTION VALUATION= COST OF PROJECT TO INCLUDE LABOR, MATERIALS, OVERHEAD.  Need SOW for Alterations or Modernizations.  Need Layout drawings for New.
