For 2008, there are changes to your health insurance options. In addition, flexible benefit deductions and your municipal benefit contribution will be calculated only in the first two paychecks of each month, or 24 times per year rather than 26.
The amount of your 2008 semimonthly Municipal Benefit Contribution will be $635.00. This contribution is provided by the Municipality to offset the cost of the Flexible Health, Life/ADD and Short Term Disability premiums.
All enrollment forms are PDF/Fillable. Each enrollment form can be completed online. Print, sign and return the form to the Benefits section. Caution: The PDF/Fillable forms can not be saved or emailed. Make sure to keep a copy for your records.
The annual open enrollment period is the only time during the year that current employees can enroll, change, increase or drop their coverage unless there is a change in family status (e.g. birth, adoption, death, marriage or divorce). Employees who have a family status change outside the open enrollment period have 90 days from the event to change their benefit coverage.
