1. A site plan drawn to scale and bearing the original stamp and signature of a registered Civil Engineer. This site plan must show:
a. The location of the on-site well and all components of the on-site wastewater disposal system. If the permit application is for the development of a new lot, the replacement disposal site shall also be shown.
b. Measured dimensions of the on-site wastewater disposal field and distribution piping.
c. Measured distances to all wastewater disposal systems, wells, surface water bodies or drainage courses, roads, property lines and structures within 200 feet of the location of any existing or proposed on-site wastewater disposal system.
d. The locations of all soils, percolation or water table tests.
e. A description of the topography, areas of excessive slope, and the extent of slopes and drainage patterns within 100 feet of any part of the system or portion of the lot intended for use for on-site wastewater disposal.
2. A design of the on-site wastewater disposal system bearing the original stamp and signature of a registered Civil Engineer.
3. The results of soils, percolation and water table tests. For systems to be constructed on newly developed lots, tests must be conducted and reported for both the original and replacement wastewater system sites. Soil tests must be logged on an approved MOA form bearing the original stamp and signature of a registered Civil Engineer.
4. A narrative description of probable impacts to adjacent properties, giving consideration to wells, wastewater systems, reserved spaces and drainage.
