Facility Use Agreement

After booking a room (see above), you may fill our, print and sign the Facilities Use Agreement. You may fax or mail the completed form to the Facility Use Coordinator. See the form for details.

The Facilities Use Policies contain useful information about the general terms and conditions of use as well as procedures for checking in, etc.
