What you need to know to Pre-Register and for your visit to our clinic:

• If you are a NEW client to our health clinic, you can get faster service by pre-registering before your visit to our clinic (at least 12 hours prior to your appointment time)
• Important - Please bring the following documentation with you:
  • Picture ID
  • Insurance card(s) such as Third Party Insurance and Medicaid
  • Income verification document(s) if you would like to be considered for discounted services:
    • Recent paycheck stub
    • Unemployment check stub
    • Current Tax return form
• Services can be discounted based on income and family size. You will not be denied services due to inability to pay
